PARENT AGREEMENT FORM

You can read this now, or you can read and sign off later if your child is offered a place.

Dear Parent/Carer,

Thank you for your interest in participating in the Perfect Pitch Project and Clear Pitch Pilot Study. This project is part of a pre-research initiative exploring how children respond to simple, engaging listening activities designed to support musical pitch recognition. The aim of this pilot is to test and refine the materials and process before a larger formal research study is conducted. This pilot is part of a broader mission to make music available to all children, regardless of barriers — in particular, offering the rare opportunity to develop the advanced skill of pitch recognition.

Please read the following information carefully:

About the Pilot

  • The program involves short, at-home listening games for your child.

  • Activities are optional, low-risk, and designed to be fun and age-appropriate.

  • The pilot is designed to be flexible, with short daily activities and a schedule that adapts to each family’s context.

  • There are no in-person requirements unless you choose to opt in later for free, optional group sessions.

Your Privacy

  • No identifying information will be collected from your child.

  • All communication is between you (the parent/carer) and the study coordinator.

  • Participation is anonymous unless you choose to be contacted for future updates or optional events.

Voluntary Participation

  • Participation is completely voluntary.

  • You may withdraw at any time with no obligation or consequence.

  • Feedback is welcome but not required.

Agreement

  • You understand the nature and purpose of the pilot.

  • You consent to your child participating in the Clear Pitch activities.

  • You are aware that no data identifying your child will be collected.

  • You understand that this is a community-based pilot designed to help shape a future university research project.
